Immunization Information

“Got Tdap yet?” – If you haven’t already, get your pertussis (Tdap) booster shot

Check in with your doctor

• Get a record of the Tdap booster shot from your doctor

• Bring a copy of your immunization record to school now!

Medications at school:

Any medication that a student needs to be administered by the health clerk during school requires a doctors authorization. Without this form your students medication will not be able to be administered.  Students are not permitted to bring medication to school and self administer or keep the medication on their person while at school without medical/doctor authorization.   Attached below is the “Medication Authorization” form.  Please print this form, read all of the instructions and bring the completed form to the front office of the site you attend.
